The Pennsylvania State Ethics Commission has issued a final adjudication in the following matters under

HARRISBURG, PA – 

Order No. 788-S (Woodson)
Income Maintenance Caseworker
Department of Human Services
Berks County, PA

ORDER SUMMARY: 

1.         Amelia Woodson (“Woodson"), as an Income Maintenance Caseworker with the Pennsylvania Department of Human Services (“Department of Human Services"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar years 2020 and 2021 with the Department of Human Services. 

2.         This Commission hereby levies two maximum civil penalties against Woodson, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day her Statements of Financial Interests for calendar years 2020 and 2021 remained delinquent, for a total civil penalty of Five Hundred Dollars ($500.00).  Woodson is ordered to pay said civil penalties in the total amount of $500.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Woodson is ordered to file a complete and accurate Statement of Financial Interests for calendar years 2020 and 2021 with the Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

 

Order No. 789-S (Robinson)
Income Maintenance Case Worker
Department of Human Services
Philadelphia County, PA

ORDER SUMMARY: 

1.         Lakeshia Robinson (“Robinson"), as an Income Maintenance Caseworker with the Department of Human Services, failed to timely file Statements of Financial Interests for calendar years 2020 and 2021 in compliance with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a).  

2.         Per the Consent Agreement of the parties, we hereby levy a civil penalty against Robinson in the total amount of $300.00 for Robinson's delinquent Statements of Financial Interests for calendar years 2020 and 2021. 

3.         To the extent Respondent has not already done so, Robinson is ordered to make payment of the aforesaid civil penalty in the total amount of $300.00 by no later than six months after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding to this Commission a certified check or money order in the amount of $300.00 made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

4.         Failure to comply with Paragraph 3 of this Order will result in the initiation of an appropriate enforcement action.

 

Order No. 790-S (Hosage)
Unemployment Compensation Tax Technician Trainee
Department of Labor and Industry
Cumberland County, PA

ORDER SUMMARY: 

1.         Andrea Hosage (“Hosage"), as an Unemployment Compensation Tax Technician Trainee with the Department of Labor and Industry, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Labor and Industry. 

2.         This Commission hereby levies one maximum civil penalty against Hosage at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Hosage is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Hosage is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2021 with the Department of Labor and Industry by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

Tax Examiner 2
Department of Revenue
Dauphin County, PA

ORDER SUMMARY: 

1.         Ashlii Randolph (“Randolph"), as a Tax Examiner 2 with the Pennsylvania Department of Revenue (“Department of Revenue"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Revenue. 

2.         This Commission hereby levies one maximum civil penalty against Randolph at the rate of Twenty-Five Dollars ($25.00) per day for each day Respondent's Statement of Financial Interests for calendar year 2021 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Randolph is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Randolph is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2021 with the Department of Revenue by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

ORDER SUMMARY:

1.         Hope Gainous (“Gainous"), as an Income Maintenance Caseworker with the Pennsylvania Department of Human Services (“Department of Human Services"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Human Services. 

2.         This Commission hereby levies one maximum civil penalty against Gainous at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2021 remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Gainous is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury.

ORDER SUMMARY: 

1.         Akilah Ingram (“Ingram"), as an Income Maintenance Caseworker with the Pennsylvania Department of Human Services (“Department of Human Services"), failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar year 2021 with the Department of Human Services. 

2.         This Commission hereby levies one maximum civil penalty against Ingram at the rate of Twenty-Five Dollars ($25.00) per day for each day her Statement of Financial Interests for calendar year 2021 remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Ingram is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Ingram is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2021 with the Department of Human Services by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

Ridley Park Borough
Delaware County, PA

ORDER SUMMARY: 
1.         Michelle Mattus (“Mattus"), as a Council Member for Ridley Park Borough, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when she failed to file a Statement of Financial Interests for calendar years 2019 and 2020 with Ridley Park Borough. 

2.         This Commission hereby levies two maximum civil penalties against Mattus, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day her Statements of Financial Interests for calendar years 2019 and 2020 remained delinquent, for a total civil penalty of Five Hundred Dollars ($500.00).  Mattus is ordered to pay said civil penalty in the total amount of $500.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Mattus is ordered to file a complete and accurate Statement of Financial Interests for calendar years 2019 and 2020 with Ridley Park Borough by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

Commonwealth of Pennsylvania
Mercer County, PA

ORDER SUMMARY: 

1.         Terrence Crumby (“Crumby"), as a Constable/Deputy Constable, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2022. 

2.         This Commission hereby levies a maximum civil penalty against Crumby at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2022 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Crumby is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Crumby is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2022 with this Commission by no later than the thirtieth (30th) day after the mailing date of this Order. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

Zoning Hearing Board and Civil Service Commission Member
Borough of Donora
Washington County, PA

ORDER SUMMARY: 

1.         Dale Shawley, Jr. (“Shawley"), as a Member of the Zoning Hearing Board and the Civil Service Commission for the Borough of Donora, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar years 2019, 2020, and 2021 with the Borough of Donora. 

2.         This Commission hereby levies three maximum civil penalties against Shawley, each at the rate of Twenty-Five Dollars ($25.00) per day, for each day his Statements of Financial Interests for calendar years 2019, 2020, and 2021 remained delinquent, for a total civil penalty of Seven Hundred and Fifty Dollars ($750.00).  Shawley is ordered to pay said civil penalty in the total amount of $750.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Shawley is ordered to file a complete and accurate Statement of Financial Interests for calendar years 2019, 2020, and 2021 with the Borough of Donora by no later than the thirtieth (30th) day after the mailing date of this Order, with one copy forwarded to the Pennsylvania State Ethics Commission for compliance verification purposes. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.

Governor's Invasive Species  Council
Chester County, PA

ORDER SUMMARY: 

1.         Michael Dunn (“Dunn"), as a Member of the Governor's Invasive Species Council, failed to comply with Section 1104(a) of the Public Official and Employee Ethics Act (“Ethics Act"), 65 Pa.C.S. § 1104(a), when he failed to file a Statement of Financial Interests for calendar year 2022 with this Commission. 

2.         This Commission hereby levies one maximum civil penalty against Dunn at the rate of Twenty-Five Dollars ($25.00) per day for each day his Statement of Financial Interests for calendar year 2022 has remained delinquent, for a total civil penalty of Two Hundred and Fifty Dollars ($250.00).  Dunn is ordered to pay said civil penalty in the total amount of $250.00 by no later than the thirtieth (30th) day after the mailing date of this Order, by utilizing the Commission's electronic payment system or by forwarding a check to the Pennsylvania State Ethics Commission made payable to the Commonwealth of Pennsylvania, for deposit in the State Treasury. 

3.         Dunn is ordered to file a complete and accurate Statement of Financial Interests for calendar year 2022 with this Commission by no later than the thirtieth (30th) day after the mailing date of this Order. 

4.         Failure to comply with Paragraph 2 or 3 of this Order will result in the initiation of an appropriate enforcement action.
